Dosage of Letermovir

Hematopoietic Stem Cell Transplantation

The recommended dose is 480 mg orally or intravenously once daily. For oral administration, the recommended dose is one 480 mg tablet or two 240 mg tablets once daily. For patients who cannot swallow tablets, four packets of 120 mg oral granules may be used once daily. Start letermovir tablets between Day 0 and Day 28 post-transplant and continue until Day 100 post-transplant. For patients at risk for late CMV infection and disease, letermovir tablets may be continued until Day 200 post-transplant. Adjust the dose of letermovir tablets when coadministered with cyclosporine.

Kidney Transplantation

The recommended dose is 480 mg orally or intravenously once daily. For oral administration, the recommended dose is one 480 mg tablet or two 240 mg tablets once daily. For patients who cannot swallow tablets, four packets of 120 mg oral granules may be used once daily. Start letermovir tablets between Day 0 and Day 7 post-transplant and continue until Day 200 post-transplant. Adjust the dose of letermovir tablets when coadministered with cyclosporine.
